Flu Shots for BK Students and Families

On Wednesday, October 17, from 7 a.m. to 9 a.m., Occupational Health, Inc. will be at Bishop Kelley in the Student Commons to administer the flu vaccine to the Bishop Kelley students, families and faculty.  As you know, people are most susceptible to contracting the flu when confined in spaces with a large number of people.  Getting an annual flu vaccine is the first and best way to protect yourself and your family from the flu. 

Every student being vaccinated must have a consent form filled out and signed before receiving the vaccine. It is attached, along with a flier containing more information about this year's vaccine. Occupational Health, Inc (OHI) will file Community Care Insurance with no out of pocket cost. If paying cash, the cost for each vaccination is $35.00. OHI accepts cash and checks only, and will provide a receipt upon request for those with non-Community Care insurance. Checks should be made payable to OHI.   In addition to the signed consent form, students will need to have a copy of their Community Care insurance card (If applicable), check made payable to OHI or cash in the amount of $35 for the Quadrivalent Shot.  If sending cash, please put in an envelope with the signed consent form with your student’s name clearly marked on the outside.  

The consent form and payment must be returned to the School Nurse in the main office by Wednesday, October 17. Parents and grandparents are welcome to come and get the flu vaccine as well.
